Filling of the lid-cheek junction and the midface with some form of dermal filler is typically what takes place to treat hyperpigmentation. Fillers can be synthetic such as hyaluronic acid agents (i.e.: Restylane, Juvederm, or Belotero) or actually acquired from the patients themselves through abdominal fat harvesting.

Fillers injected to the lower lids can certainly help improve the volume deficiency people get with aging, but it will also reduce the amount of herniated fat visible to others. Fillers can also help improve the hyperpigmentation that is often present in such cases. I prefer using dermal fillers Restylane or Belotero or fat grafting to treat this area.

This is done in the office with the assistance of a topical anesthetic. The procedure can take as little as 30 minutes if using Restylane and patients usually go home afterward without any problems. I have introduced the use of a blunt-tipped cannula for most of my facial filler injections, I offer this to all my facial filler patients and most if not all prefer the blunt-tipped cannulas due to ease of use for patient comfort, safety, and decreases the rate of bruising.
Not only do I get wonderful results, but the patients are extremely comfortable during the procedure with just the use of a topical anesthetic.
The blunt-tipped cannulas I now use for my facial filler injections are similar to the ones I use in the operating room for my fat grafting procedure, which has made the transition to using the cannulas in the office quite feasible.
